How do I create a index to increase the performance on Key Lookup

Two easiest methods: 1) Modify the query to join on the clustered index. This is not always possible. 2) Take the columns in the Key Lookup "output list" and add them as include columns to episode.idx_episode_podcast_includeplublication date. This makes your index a little larger (carrying along 8 extra fields per index leaf), which will affect performance a little. You may not even notice the performance hit, that's all part of testing.
